D&M Supported Living aims to provide support to enable older people 18 to 65 to gain stability, increase their independence, facilitate positive outcomes and fulfil their housing and support-related needs.
They aim to provide a safe, stable, nurturing and stimulating environment for their service users and vulnerable adults. The rights and dignity of all their service users will be paramount; living skills will be developed to enhance their quality of life and to promote independence.
They provide:
- Support to maintain independence with activities of daily living.
- Support with personal care, such as bathing, washing and shaving.
- Supporting getting up or going to bed.
- Helping people to continue to live in their own homes and enable them to maximise their independence.
- Support with medication, ordering prescriptions etc.
- Support with preparing nutritious and healthy meals.
- Support with eating and drinking, encouraging food and fluid intake.
- Support with continence management and toileting.
- Light housekeeping.
- Medication.
- Shopping.
- Money management.
- Companionship.
- Social support.
Referral required and eligibility
- Their service is available for 18 to 65. Referrals for their service must come from local authorities and/or other organisations.
