Proof of automatic entitlement

Award letters should include all pages, and:
- your name and address
- the date the letter was issued
- the correct qualifying award/component
- the end date of the award*
*(Note: Your badge will be issued until the end date on the award letter or for a maximum of 3 years, whichever is shorter.)
Plus, any one of the following:
Proof of Disability Living Allowance (DLA)
A copy of your entitlement to the higher rate mobility component of Disability Living Allowance (DLA). This letter must be dated within the last 12 months. This letter must confirm your name, address, date, level and duration of your award and how your allowance is made up. If you do not have a current entitlement letter you can obtain one from Department for Work and Pensions on 0800 121 4600.
Proof of Personal Independence Payment (PIP)
A copy of your entitlement letter showing you receive 8 points or more under the ‘moving around’ activity of the mobility component of Personal Independence Payment. This letter must be dated within the last 12 months. Please ensure you provide page 1 of this letter along with the page showing the points for the ‘moving around’ activity. If you do not have a current entitlement letter you can obtain one from the Department for Work and Pensions on 0800 121 4433.
