Depending on the criteria you are applying under, you would need to send one of the following:

• a letter detailing the type of medical equipment needed for your child. This must be from a healthcare professional
• a letter detailing the medical condition affecting your child. This must be from a healthcare professional and must outline the need for your child to be kept near a vehicle at all times so they can, if necessary, be treated in the vehicle or quickly driven to a place of medical treatment, such as a hospital
